You are a caregiver if you....

(such as bathing, dressing, toileting, meal or feeding task, aiding walking/movement).

bulletPerform task to help

(such as errands, shopping, budgeting, medical appointments, home repairs and chores, and transportation).

bulletSeek assistance and/or formal services

(i.e. hiring or needing someone to stay with your loved one).

bulletConsider long-term care or placement

(making the decision to place your loved one and then your level of involvement after placement).

bulletCoping with loss

(when death, end of life issues, or just a change in the relationship brings about difficult adjustments for you).

Family Caregiver Support  Program (FCSP) serves.....

bulletAny adult family member or another individual who cares for a loved one who is 60 years of age or older in a home setting.
bulletOlder grandparent or older person who is 60 years of age or older and is the primary caregiver of a child who is 18 years of age or younger.

The FCSP priority populations are...

bulletOlder individuals with greatest social and economic need
bulletOlder individuals (60+) caring for children (18 or younger) who are mentally challenged/developmentally delayed

 

The FCSP can help within existing county resources...
  1. Information and Referral Services
  2. Help receiving and understanding the process to access services
  3. Counseling and Education (i.e. Caregiver trainings, educational seminars)
  4. Respite Care (giving a caregiver a break)
  5. Supplemental Services (i.e. grab bars, safety checks)
