 | Adult Day Care provides an
organized program of services during the day in a community group
setting for the purpose of supporting the personal independence of
older adults and promoting their social, physical, and emotional
well-being. Programs must offer a variety of activities designed to
meet the individual needs and interests of the participants, including
referral to and assistance in using other community resources. |

 | Adult Day Health services are
similar to adult day care programs in that they provide an organized program of services
during the day in a community group setting to support the personal
independence of older adults and promote their social, physical, and
emotional well-being. In addition, providers of adult day health
services, as the name implies, offer health care services to meet the
needs of individual participants. |

 | Congregate Nutrition is a service
where a meal (typically lunch), offering one-third of the Recommended
Daily Dietary Allowance, is provided in a group setting. |
|
 | Family
Caregiver Support Program
The enactment of the Older Americans Act Amendments
of 2000 (Public Law 106-501) established an important new program, the
National Family Caregiver Support Program (NFCSP). The program
calls for all states, working in partnership with area agencies on
aging and local community-service providers to have five basic
services for family caregivers, including:

Information
to caregivers about available services;

Assistance
to caregivers in gaining access to supportive services;
|
 |
Individual
counseling, organization of support groups, and caregiver training
to caregivers to assist the caregivers in making decisions and
solving problems relating to their caregiving roles;
|
 |
Respite
care to enable caregivers to be temporarily relieved from their
caregiving responsibilities; and
|
 |
Supplemental
services, on a limited basis, to complement the care provided by
caregivers.

 | Information and Assistance (I
& A) services are designed to:
 | assess and evaluate an individual's
needs; |
 | inform and educate about programs
and services available across the long-term care continuum;
|
 | refer and/or directly connect the
individual to appropriate resources; |
 | provide assistance to negotiate the
service delivery system; |
 | work with long distance caregivers
in identifying and locating needed services; and |
 | advocate on behalf of individuals or
a group to obtain change in the delivery or availability of
services. |

 | Mid-East Commission operates an
automated information and referral data base of services and resources
for seniors and their caregivers at http://www.aginglink.org. |
|
 | In-Home Aide involves the
provision of paraprofessional services that assist functionally
impaired older adults and their families with essential home
management and personal care and/or supervision to enable the older
adult to remain at home as long as possible. |
|
 | Long
Term Care Ombudsman Program consists of state and regional
ombudsmen who help residents of long term care facilities to exercise
their rights. In addition to being an advocate for residents, they
educate the public and facility staff about rights and help resolve
grievances between residents/families and facilities. |
|
 | Legal Services is a service for older
people who sometimes need legal assistance. For those who cannot
afford to pay privately for this assistance, Legal Services of North
Carolina, Inc. (sometimes called Legal Aid) is a possible resource for
civil (not criminal) matters. This network of non-profit law offices
serves eligible citizens across the state. Some private attorneys also
volunteer time through pro bono programs, often in cooperation with
Legal Services offices. |

 | Transportation is a service that
provides travel to and/or from community resources such as medical
appointments and nutrition sites or other designated areas for older
adults needing access to services and activities necessary for daily
living. |
|
 | Senior Center Operations is the operation
of a multipurpose senior center that includes the provision of a broad
spectrum of services and activities for older adults. The primary
objectives of a multipurpose senior center are: the centralized
provision of services that address the special needs of older adults;
opportunities for older adults to become more involved in the
community; and, the prevention of loneliness and premature
institutionalization by promoting personal independence. |

 | Volunteer Program Development is
the development and operation of a systematic program for volunteer
participation. The service is intended to involve volunteers of
all ages in providing services to older adults while also providing
community service opportunities for older adults. |
